New AFT members are eligible for no-cost $5,000 group term life insurance and are required to complete a separate application to obtain the insurance.

• Local union leaders may obtain bulk quantities of pre-paid return-addressed application forms from the CFT by requesting them below.

• You may also download an application directly from the insurance company and return completed applications to the address on the form.

• Please note: If you have AFT+ applications for the free $5K life insurance on hand, please make sure they are current and contain the following language: “I understand that my coverage will become effective on the first day of the month following the date this application is signed and received by the administrator.” (The word “received” must be part of this sentence.)​
